To begin your Fear Free visit, we’d like to know more about your pet and how they behave at home, on the ride and during their vet visit. This is valuable information to help us tailor our techniques for each animal we see. You can fill out our Pre-Visit Questionnaire (PVQ) via the links below. It is important to fill out a PVQ for each pet, and we will keep it on file for 12 months. Don’t worry, we’ll send you a reminder when it’s time to fill out an update.
